Post-Secondary Transition Plan (PTP): Best Practices

This webinar will go through the process of the Postsecondary Transition Plan also known as the PTP. It will also provide tips and ideas to assist parents and youth with how to interact during the PTP discussion as well as how to get youth involved in their PTP meeting.


Pam Jenson is the Project and Data Coordinator for the statewide Transition Improvement Grant (TIG). Pam’s educational background includes a Bachelors and Masters Degree in Vocational Rehabilitation from the University of Wisconsin-Stout. For over 20 years, Pam has worked with individuals with disabilities in various transitional capacities including: residential, case management, school to post-secondary options, and vocational evaluation and training environments. She has consulted youth, teachers, parents, agency providers, businesses and community members regarding the pathways and benefits of transition, accommodations, and acceptance of persons with disabilities
