This session will cover the new WiTransition appthat is available to schools, youth, families, and agencies to provide hands-on access to creating and having input into a transition plan for a student. We will review the app content, the report that is generated at completion, and tips on how to best use the app to move transition planning forward for Wisconsin youth.
Nancy Farnon-Molfenter has been working in the field of disability services for more than 25 years and strives to support inclusive outcomes for all people. She earned a Masters degree in Special Education from Northeastern University and a Ph.D. from the University of WI-Madison in Special Education and Rehabilitation Psychology. Nancy has been employed as a special educator, transition coordinator, and administrator for both employment and home support agencies. She also worked as a project consultant for the WI BPDD with the Let's Get to Work and Employment First state-wide projects. Nancy now serves as an Educational Consultant and Transition Improvement Grant Director at the Department of Public Instruction. In addition, Nancy teaches courses in the UW system.
Brenda Swoboda is the Western Regional Transition Coordinator and supports CESA's 4.10 and 11 under the new Transition Improvement Grant (TIG). She graduated from the University of WI-Stout with a degree in Vocational Rehabilitation and Concentration in Independent Living. She then received her Master's Degree in Education at UW-Stout as well, along with her Cognitive Disabilities teaching license for grades 7-12. Prior to coming to the TIG, she taught special education for 17 years and was a Transition Coordinator for 6 of those years. Brenda is passionate about transition for our students with disabilities and believes in "thinking outside the box" when creating transition plans. Her office is located at CESA 4 in West Salem, WI.
