Our focus for this SDS Network Forum is on Wisconsin’s first long-term
care outcome, “I Choose Where and With Whom I Live.”
There is little in life more important to any of
us than our home and what it
means to us as the center from which our life flows. Come and learn how
SDS can help in enabling people to have a home in which they are at the
center, own or lease where they live, choose who lives with them, and
obtain,
create or purchase needed support from others.
We have many exciting speakers to lead us through the day!
Lyle and Mary Romer from Total Living Concept in
Washington State will open this event with a morning general session, plus
afternoon breakout sessions with the theme of We All Need a Home. Total Living Concept
began in 1982 operating group homes that provided congregated
living options for people with developmental disabilities. Through the process of
constant self-evaluation and listening to the people being supported, TLC
gradually moved away from congregated housing
options to providing individualized support enabling each person to live
in their own home.

feature developing Supported Living in Wisconsin; housing development with
Movin’ Out; how PEONIES helps us think about the theme of this Network;
housing accessibility, and more!
The National Youth Leadership Network is excited to announce the
Reap What You Sow: Harvesting Support Systems Institute.
This event is for young people, parents, siblings, friends, allies,
teachers, community support providers, and more. It will take place
July 29-31, 2010 in Raleigh North Carolina. This
conference will be taking place at the
North Raleigh Hilton.
The Institute centers on a curriculum [a set of learning activities]
that a team of young people with disabilities created. (See: About
the Reap What You Sow: Harvesting Support Systems curriculum.)
The Institute is for people who want to learn about the curriculum and
those who may be interested in teaching it in their communities. The
Institute will include workshops and activities. People attending the
Institute will also learn about ways to engage youth and family members in
their communities.

Financial assistance is available to
young adults with Down syndrome who want to enroll in post-secondary
programs or take classes that will help them with employment,
independent living and life skills. To date, over thirty students have
received grants from the National Down Syndrome Society (NDSS) to help
cover the costs of pursuing their post-secondary education and
enrichment goals. The 2010 application form can be downloaded in PDF

The Natural Supports Project at the University of Wisconsin-Madison is
looking for youth and young adults ages 14-18 with developmental
disabilities who are currently employed in community-based,
competitive-wage jobs to be featured in a set of transition stories.
Youth who are chosen to participate will receive a $25 gift card.

The 9th AnnualWI Youth Leadership Forum (YLF)
is a FREE program for high school students with disabilities
who would like to take part in a unique and fun week long leadership
development program. Participants will sharpen their leadership and
self-advocacy skills, develop their career awareness, experience college
life, meet successful speakers from across the state with similar
disabilities as theirs and develop their own Personal Leadership Plan that
they will implement when they return home.
The YLF will be held June 27 – July 2, 2010 on the
Edgewood
College
campus in Madison, WI.
APPLICATION DEADLINE: MARCH 1, 2010
The YLF is a competitive
process. Applicants must complete the application, submit a brief
response to 2 essay questions and obtain 3 letters of recommendation.
The Model
Communities project, funded by the Wisconsin
Medicaid Infrastructure Grant
through the Department of Health Services is looking for people with
innovative ideas on how to make their community more welcoming and
inclusive of all people, including people with disabilities. The
goal of the Model Communities project is to discover promising
approaches to include people with disabilities more fully and
naturally
in school, work, and community activities. We believe that when
all people in the community are included, relationships are built that
lead to a better quality of life.

Scholarship applications now being accepted for the
2010 Wisconsin Transition Conference
Parent and youth scholarship applications are now
being accepted for the 7th Annual Wisconsin Transition Conference on
February 18-19, 2010 at the Kalahari in Wisconsin Dells. To be eligible
for the scholarship you must be a youth with a disability between the ages
of 14-21 or a parent of a youth with a disability between the ages of
14-21.
The application will ask you to verify the age and
disability information. All disability related information will be kept
confidential. The scholarship award pays for registration and
hotel costs for Wednesday, February 17th and Thursday, February 18th.
Included in the registration costs are materials and breakfast, lunch,
afternoon snack and pizza social on Thursday and breakfast and mid-morning
snack on Friday. All other expenses including transportation, meals on
Wednesday and Friday after the morning will snack will be the
responsibility of the scholarship recipient.
