Bridging the Gap: Health Care Transition for Community Partners

As adolescents move in young adulthood is seems like their whole lives are in transition. One transition that is foundation to success and often overlooked is the transition to the adult health care world. Starting as early as 12 years old, youth and families can begin to learn skills and build awareness so the youth can take charge of their own health care as an adult.

Learning Objectives:

  • Understand what Health Care Transition (HCT) is
  • Learn how HCT is foundation to success in life
  • Apply HCT to personal circumstances


Tim Markle is a curriculum developer and speaker for the statewide Youth Health Transition Initiative our of the Waisman Center. He also is the parent of an adult 23 year old on the autism spectrum and continues to work on transitioning.
