Wisconsin Transition App

This session will cover the new WiTransition app that is available to schools, youth, families, and agencies to provide hands-on access to creating and having input into a transition plan for a student. We will review the app content, the report that is generated at completion, supporting materials that have been developed and tips on how to best use the app to move transition planning forward for Wisconsin youth.


Brenda Swoboda, the Western Regional Coordinator for the Transition Improvement Grant (TIG), graduated from the University of Wisconsin - Stout with a degree in Vocational Rehabilitation and Independent Living. She then went on to receive her Master’s Degree in Education at the University of Wisconsin – Stout receiving her license to teach students with Cognitive Disabilities. Prior to coming to the Transition Improvement Grant, she was a special education teacher for 17 years and a transition coordinator for 6 years. Brenda is located at CESA 4 in West Salem, Wisconsin and covers CESA’s 4, 10, and 11.
