WEBINAR DESCRIPTION: This session will cover the new WiTransition app that is available to schools, youth, families, and agencies to provide hands-on access to creating and having input into a transition plan for a student. We will review the app content, the report that is generated at completion, supporting materials that have been developed and tips on how to best use the app to move transition planning forward for Wisconsin youth.

PRESENTER: LaNae Jabas is the Central Region Coordinator for the Transition Improvement Grant (TIG) and supports districts in CESAs 5 & 6. LaNae brings fifteen years’ experience ranging from vocational rehabilitation to secondary education. She is an enthusiastic and energetic individual who wholeheartedly believes in transition and the positive impact that a “team approach” has in creating and implementing a transition plan for students with disabilities.
