This updated, interactive app mirrors the Postsecondary Transition Plan (PTP) in youth- and family-friendly language, making it easier than ever for students and families to engage in the process. Discover the new resources that accompany the app, designed to support educators in guiding meaningful, student-led transition planning. Participants will leave with practical strategies to integrate the app into their workflow, boost student and family engagement, and make transition planning more effective, collaborative, and fun.

Presenter:

Pam Jenson is the Program Coordinator for the statewide Transition Improvement Grant (TIG). Pam’s educational background includes a Bachelors and Masters Degree in Vocational Rehabilitation from the University of Wisconsin-Stout, with a concentration in the areas of Rehabilitation Counseling and Vocational Evaluation. For over 25 years, Pam has worked with individuals with disabilities in various transitional capacities including: residential, case management, school to post-secondary options, and vocational evaluation and training environments. She brings her expertise, enthusiasm and excitement to ignite conversation around innovative and effective strategic transitional planning that will provide opportunities for all students to be college, career and community ready.

  • Increase family and youth engagment in the transiton planning discussion during a students IEP meeting.
