Description:

The special health care needs or chronic health conditions of students do not disappear once the student graduates from school. Schools can work with families, students, and healthcare providers to include planning for the transition of student needs as they move into, through, or out of the school setting. This presentation will discuss ways parents can work with schools to include health care transition planning into IEPs to foster development of self-care skills and active participation in decision-making.

 

Presenter:

Louise Wilson is an accomplished nursing leader with over 32 years of experience in school health services and 45 years in Nursing. As the State School Nurse/Health Services Consultant for the Wisconsin Department of Public Instruction (DPI) she provides state-wide leadership in the areas of school health, students’ physical and mental health, school nursing, and student health services in general. Louise is a Nationally Certified School Nurse, holds a voluntary DPI license in School Nursing, and is a Fellow in the National Academy of School Nursing.

 

Learning Objectives:

Describe the role of school nurses to support health care transition planning for students with disabilities or special health care needs.

Identify strategies parents and healthcare professionals can use to support schools and school nurses in student transition to adulthood.
