Description:

This session is a broad overview of the transition to adult life for our soon-to-be-adults with special health care needs or disabilities. We’ll share hints and tips so you can start planning for the aspects of life that often change at adulthood including at school, in health care settings, and in long-term support programs. We’ll also explore alternatives to guardianship including supported decision-making and other legal options and tools.

Presenter:

Danielle Tolzmann currently works as Director of Family Voices of Wisconsin, and serves the broader community on multiple councils, boards and committees. She is also parent to a young adult at home with special health care needs, and is an active grandparent to neurodiverse grandchildren.

Learning Objectives:

A broad overview of the environmental and legal changes that occur with the transition to adulthood; hints & tips to plan for life after high school; gain tools, resources, and new insights for thought as you plan for our child’s adulthood phase of life.
